You mean like a cap for inside of the fitting right? At home depot they sell ones but they aren't pvc theyre plastic so when I used one I cut a peice of sheet metal the size of the hole and put calk around the edges then put the sheet metal in and calked that. The peice of pvc will push up against the cap and metal and keep it in place. It holds for my combustion but if your planning on making a pnuematic I'm not sure.

Well I don't know I guess you should just use multiple layers of sheet metal and just skip the plastic cap since the metal would be stroger but I'm not sure on this so wait until someone else posts but I think maybe like 4 or 5 layers would do it.

drill out two reducers, so the pipe can slide through, cap the pipe you put inside of it. Set up the reducers like -}{-

Reducers or busings? I don't follow your post Brian and even though I'm not a veteran at this I don't belive he will either.
*edit*
Try looking at a specialty plumbing store, the one I buy all my stuff at has caps that have the O.D. the same as most I.D. pipes.

take a bushing, a reducer you put on the outside, a bushing on the inside. The work like BTB said.
