Whatever caused it...it would be better to have a little more clearance to prevent it and have O-rings to overcome the issue.
It is a blast to lathe or shape on a drill and pretty tough.

Its not that the wood swelled up and jammed (it was varnished), but rather the O-ring groove could not be machined fine enough to make a good enough sealwood works just fine, you just have to seal the wood with pea glue or varnish to stop it swelling and binding
