Hmmm... I don't recall any such example.
Would the adibatic heating from the gases compressing between the pellet and valve cause that? Including the steel oxidising rapidly and lubricant igniting naturally.
It could be any manner of ignition sources, remember in a pressurised oxygen environment most things - even those not normally flammable - are incredibly flammable, so even striking the container against something might provoke a disaster.
