Hard to say really. Obviously it wasn't the warhead because the explosion wasn't strong enough

there are two theories that appear most often - he accidentaly hit the wall or somehow the round got stuck in the launcher
Personally I think it's the first one as it seems that the launcher is shot backward (recoils) whereas if the round was stuck it would be recoil in the opposite direction

What I find surprising is that there is so little blood - almost looks as if his wound was not caused by force of the launcher ripping it apart but booster gasses... I do know there's a paratrooper version where the tube is in two parts that are joined together but no idea if that's the case

IDK we'll see - possibly JSR knows something about it

My hypothesis would be that there was something in the blast tube and he took the launcher to the shoulder. It doesn't look like there was sufficient distance for the warhead to arm and that I believe is only around 5 metres. Ineptly cleaning thing beforehand might have had something to do with it, if the blast tube was clogged then the recoilless effect of the booster charge is lost - and firing a 2kg warhead at 115 metres per second... that's the recoil force of more than five 50 caliber rifles fired at the same time, from a very light launcher.

There are quite a few own goals where they hit short, this one for example, and the warhead blast is much bigger.

I've been looking for answers to this on several forums with no luck. I am leaning towards my rpg7d hypothesis - if you compare pictures of standard & paratrooper versions and then with the video it seems it was indeed rpg7d not the regular one. See for yourself - I see an RPG7d there

I doubt this would cause the type of injuries seen - the booster charge is not high explosive, it's relatively slow burning and would not cause the sort of massive overpressure to create such a wound. Granted, the 7D comes apart at the point where he rests it on his shoulder, but if you look at it frame by frame there is no evidence of flames or smoke coming out from that area.

I think they are perfectly safe for the operators, providing they are properly trained and the weapons themselves are manufactured with the proper quality control and safety devices.

New forging hammer! Well, "new" for me :D Found the head (don't ask where) and it seems to be very hard and old. The head weights 4 kg and I made an oak handle for it today. Now i just need some coals to try it out...

From the forest near my house. The sledgehammer was used by my father in the late 80's when he was building our house and the handle broke and well.... I think he has some anger management problems :D

For certain values of "common" that include 13mm Gyrojet, I see.
